Healthwatch Sheffield wants to hear about people’s experiences of mental health services whilst going through a crisis.
Healthwatch Sheffield wants to hear about people’s experiences of mental health services whilst going through a crisis.
We have created a short survey (25 questions which are mostly multiple choice) to help capture your views about the support and treatment you received.
We would like anyone who has experienced a crisis, as well as family members, friends or advocates who care for someone with a mental health condition, to take the survey.
Any personal information given will remain confidential in accordance with the Data Protection Act 1998.
We will also anonymise any information we use to help us write up a report of our findings.
